All forms completed by  the provider are subject to a $25.00 processing fee per form or letter.

Annual Physicals

Q: What should I expect during my Annual Exam?

A: An annual exam is a once-a-year visit to your primary care provider for a general health check. It generally lasts about 30 minutes. During this appointment you can expect your physician to review the following:

  • General physical exam (blood work and EKG)
  • Update of family health history (any new serious illnesses in your family?)
  • Review of your health history
  • Update of current medications, herbs, and supplements
  • Need for medication refills
  • Evaluation of need for health screening tests based on age and personal and family history (such as mammogram, test for sexually transmitted diseases, and colon cancer screening)
  • Update on immunizations
  • Schedule follow-up visit to discuss new health concerns or address chronic health issues.

Q: Does my insurance cover an Annual Exam?

A: While most insurance companies cover one annual exam a year, it is important that you check with your insurance carrier to review your specific policy. Please note that Medicare does not cover routine annual exams. You may choose to pay for routine physicals and tests out-of-pocket or by using a flexible health-spending plan that your employer may provide.

Check in Time and Lab Results

All Co Pays, Co Insurance & Deductibles are due upon check in.

  • New patients: Patients with Completed new patient forms must arrive 25 minutes prior to scheduled appointment time. Patients without Completed new patient forms must arrive 35 minutes prior to scheduled appointment
  • Established Patients: In order to provide service in a timely manner, we are depending on your help. We encourage all patients to arrive 5 to 10 minutes prior to the scheduled appointment time for a hassle free check in.
  • All Patients: To avoid delays in the schedule, we may have to reschedule patients that arrive later than 10 minutes after their scheduled Appointment time.

Lab Results

Dear Patient

It creates extra administrative work and disrupts the workflow of our every day affairs to answer phone calls regarding lab results.

To better serve our patients and to avoid confusion we decided to state the following policy: Our providers will discuss your lab results during a follow up visit, this provides accurate and clear information after thorough evaluation of the your lab results.

Our Providers will then provide you with the adequate treatment plan according to the results. However if there is an immediate need or an emergency, feel free to call our office during business hours. Within two full working days we will process your request and call you back.
